ARCANUM FACILITY USAGE AGREEMENT

I, the recipient, am entering into this agreement with Community of Faith churches while I use a COF facility located at:
On the following date:
I have read, understand, and will comply with all the policies and guidelines outlined in this agreement.
Now, therefore, the recipient agrees to the following:

POLICY

Church facilities can be used by those who attend the church with the criteria set by Community of Faith Churches. Costs associated with broken, damaged or missing property, as well as any cost incurred from disarray (in excess of what's covered in the cleaning fee), will be charged to the recipient.

FACILITY GUIDELINES

The site contact person:
1. The site contact person will be present at all times.
2. No church supplies are to be used without permission.
3. No advertisements are to be distributed in the parking lot areas.
4. Alcohol and/or smoking are prohibited on church premises.
5. Facilities may not be used for commerce for the financial gain of an individual nor organization.
6. Under no circumstances are children to be left unattended. Parents much accompany all children under the age of 13 at all times.
7. Kitchen facilities may be used.

DECORATIONS

1. Candles may be used only if contained in glass. The flame must not exceed the top of the container. All taper candles used must be dripless.
2. Small items such as confetti, glitter, etc. are not allowed in the building due to difficulty in clean-up.
3. No nails, push pins, or staples are to be used on any walls or furniture.

MAINTENANCE AND UPKEEP

1. Recipient is responsible for the set-up and teardown of tables, chairs, and equipment.
2. All decorations, table coverings, coffee, cups, etc. are the responsibility of the person who has signed the Rental Agreement.
3. Recipient is responsible for ensuring the facility is left as it was originally found.

TECHNICAL

1. All technical needs such as sound system, microphones, tape players, TV's, etc. must be requested.
2. Sound/Technical needs are to be handled by COF personnel only. Individuals, other than COF personnel, are not permitted to run technical or sound equipment or provide assistance.

USAGE FEES

1. Full amount of usage fees are due at least two weeks prior to the event date.

OTHER

1. The Church will have sole discretion in determining what measures are necessary to promote the health and safety of the public at the event.
2. The Recipient acknowledges that the Church is not responsible for the event in any manner, and the Recipient will indemnify and hold harmless the Church, its agents and employees, against all claims, demands, damages, injuries, liability, cost and expenses, including attorney fees, that may arise or be incurred from or related to the event.
